Residential Rehabilitation Services and Supported Housing
Our goal is to provide safe and affordable housing to adults with severe and persistent mental illness. Residential services are available in Wicomico, Worcester, and Somerset Counties. The goal of the program is to provide services to assist individuals with developing and maintaining the highest level of independence as possible. We currently provide supervised, unsupervised, and supported housing to 150 people. There are 54 supervised beds where Residential Counselors sleep overnight in the home. We also have 36 unsupervised beds where Housing Mentors assist the members during the afternoon and evening. The organization also provides Supported Housing to 60 members and the organization acts as landlord and rents apartments/rooms to members.
Staff provides varied levels of support to assist the members in achieving goals including but not limited to; budgeting, shopping, meal preparation, treatment planning, and medication monitoring. The Lower Shore Clinic partnered with Terrapin Pharmacy and added Medherent machines in 2018 in every supervised house bedroom to provide medication monitoring. It has proven to provide a more accurate way to monitor medications. It automatically prompts a member to dispense medications per his or her up-to-date dosing schedule accessed via digital pharmacy records. Makes the member's medications available when he or she activates the dispensing device within the prescribed dosing time. Reports critical clinical adherence data in real time. Notifies staff in real time if a member failed to access his or her medications, allowing them to intervene immediately. It also tracks medication utilization trends.
